REQUESTING COPY CIVIL REGISTRY DOCUMENTS ON SECPA (SECURITY PAPER) THRU BREQS (BATCH REQUEST ENTRY SYSTEM)

Who may avail?                 General Public

What are the requirements?          
*      Photocopy of Birth Certificate, if any or
*      Baptismal Certificate, or
*      Voter Registration Record

Steps:
1.    Request. Fill up Application Form to Window 2.
  1. Payment of Fees. Client will be instructed to pay a service fee of P300.00 to the Municipal Treasury.
  2. Submission of Form and NSO Payment. Client will submit the application form together with the Official Receipt and cash payment of P140.00 to the BREQS Aide.
  3. Issuance of Acknowledgement Receipt and Claim Slip. The BREQS Aide shall issue an Acknowledgement Receipt and Claim Slip of the requested civil registry documents.
  4. Encoding to the BREQS Software, Batching and Submission of Application forms for CRS Outlet of the NSO-RXI for verification and printing of requested civil registry documents. The BREQS Aide shall encode to the BREQS software the application forms. After two (2) weeks the encoded data will be batched, saved in a flash disk and forwarded to the NSO CRS Outlet in Davao City for verification and printing.
  5. Release of SECPA. The client shall present to Window 2 the claim stub. The BREQS Aide shall retrieve the SECPA and release to the client. The client shall sign in a log book.

REGISTRATION OF BIRTH AND MARRIAGE CERTIFICATES
From MONDAYS to FRIDAYS, 8AM-5PM

Who may avail?                 General Public

What are the requirements? Certificate of Live Birth (COLB) or Marriage Contract

Person Responsible:            ROSABELLA N. LUGNASIN – Receiving Clerk

Steps:
1.   Presentation Document. Present documents to the Receiving Clerk for registration (Window 3).
2.   Verification. The receiving clerk shall forward the document to Window 2 for CRIS (Civil Registry Information System) Verification whether or not the same has previous registration.
3.   Payment of Fees. Client will be instructed to pay a service fee of P50.00 to the Treasurer’s Office.
4.   Examination of Document. Receiving Clerk shall examine the document, whether it is submitted on time/delayed, supporting papers have been attached and the entries are properly filled-up.
5.   Registration of Document. Client is advised to wait while the Receiving Clerk registers the document and assigned Registry Number.
6.   Signature of Registered Document. MCR signs the registered civil registry documents and release to client.
7.   Releasing copies of documents to the client. Two (2) copies of the document shall be released to the client. The client will be instructed to sign in a log book before receiving copies of the document.

Fee              : P        50.00 for on-time registration of Birth
100.00 after 1 month for late registration of birth
150.00 after 6 month for late registration of birth
200.00 after 1 year for late registration of birth
  50.00 for on-time registration of Marriage
  20.00 per month after 30 days for late registration of marriage

REGISTRATION OF DEATH CERTIFICATES

Who may avail?                 General Public

What are the requirements?
*      Death Certificate
*      Barangay Certificate (for late registration)

Steps:
1.   Presentation of Documents. Clients shall present to the Receiving Clerk the duly filled up and duly signed Certificate of Death prepared by the Municipal Health Office. It should be signed by the Informant, by the Attending Physician and by the Embalmer, if the body is embalmed.
2.   Examination of document. The Receiving Clerk examines the documents presented if it is submitted on time/delayed and properly filled-up and signed by the concern authorities.
3.   Payment of Fees. Client is advised to pay the corresponding fees at the Municipal Treasury, if registered late.
4.   Registration of Document. The receiving clerk shall register the document, affix Registry Number.
5.   Signature of Registered Death Document. The LCR signs the registered death certificate.

Fee              :         FREE for on-time registration
20.00 per month for late registration

Duration        :         20-25 minutes

APPLYING FOR A MARRIAGE LICENSE
From MONDAYS to FRIDAYS, 8AM-5PM

Who may avail?       Male and Female of Legal Age

What are the requirements?
*      Certified True/Xerox copy of birth certificate of applicants
*      Pre-Marriage Counseling Certificate
*      Parental consent if applicant is 18 yrs. old but below 21 yrs. old
*      Parent Advice if applicant is 21 yrs. old but below 25 yrs. old
*      At least one of the contracting parties must be a resident of Banaybanay

Person Responsible:  RAFAEL M. FRIANELA

Steps:
1.   Application. Present the required supporting documents.
2.   Examination of Requirements.  Assigned Clerk examines submitted supporting documents.
3.   Preparation of Application. Assigned Clerk types the application. Client is advised to review and check the information on the prepared application. Applicants and parents signs in the application and consent/advice. 
4.   Payment of Fees. Client is advised to pay the required application fee to the Treasurer’s Office.
5.   MCR Subscribes the Application. Client is advised to bring the document to Mun. Civil Registrar. MCR interviews the applicants and parents. MCR subscribes the application. Client is advised to come back after ten (10) days publication period.
6.   Release of License. Assigned Clerk prepares the license. MCR signs the license. Assigned Clerk releases the license to the applicants.

                                           P200.00 if one party is not a resident of the municipality
                                                P500.00 if one party is a foreigner

REGISTRATION OF LEGAL INSTRUMENTS/LEGITIMATION OF NATURAL CHILD

Steps:
1.   Presentation of Document. Present the required documents to the assigned clerk.
2.   Examination of Documents. Assigned clerk examines the presented documents for registration and annotation.
3.   Payment of fees. Client is advised to pay the required fees at the Treasurer’s Office.
4.   Records the Request. Assigned Clerk records the request to logbook.
5.   Registration of the Legal Instruments and Annotation to the Affected Civil Registry Record.  Client is advised to wait while assigned clerk enters the legal instrument to Registry Book and annotates the same to the affected record.
6.   Preparation of Annotated Record. Client prepares the annotated civil registry Record.
7.   Release of Documents. Assigned clerk releases the documents to the client.
