Steps:
1.
Filing/Recording of Request. Fill up and submit
Application/ Assessment Form, along with all requirements at the Mun.
Treasurer’s Office.
2.
Review/Evaluation of
Application. MTO
Staff review & evaluate Business Application.
3.
Signing of
Regulatory Agencies (SEC/CDA/DTI/SSS/PHILHEALTH) & Issuance of Order of Payment. Clients shall
proceed to concerned offices for signature.
4.
Assessment &
Payment of Taxes. MTO
Staff reviews and approves assessment; and affixes her signature on the
printout form.
5.
Signing and
Recommending Approval by BPLO and forward to the Mayors’ Office. BPLO will sign &
recommend for approval of the LCE.
6.
Approval and Signing
of the Mayor & Release of Mayors’ Permit. Mayor approves &
releases the Business permit.

Preparation of Clearance or Certification. MO staff prepares
the Clearance or Certification.
3.
Approval of the Mayor. Mayor approves the
Clearance or Certification.
4.
Release of Clearance or Certification. The Clearance or
Certification is released to the client. Client leaves 1 copy with the
Records/Archives Section for file.
