APPLICATION/RENEWAL OF BUSINESS PERMIT/LICENSE
From MONDAYS to FRIDAYS, 8AM-5PM

Who may avail? Any person/entity engaging business w/in the municipality

What are the requirements?
*      Barangay Clearance
*      Police Clearance
*      Zoning Clearance
*      Latest Community Tax Certificate (CEDULA)
*      SEC/CDA/DTI/SSS/PHILHEALTH Clearances

Person/s Responsible:                   MA. LINDA T. DALAPALAN

Steps:
1.   Filing/Recording of Request. Fill up and submit Application/ Assessment Form, along with all requirements at the Mun. Treasurer’s Office.
2.   Review/Evaluation of Application. MTO Staff review & evaluate Business Application.
3.   Signing of Regulatory Agencies (SEC/CDA/DTI/SSS/PHILHEALTH) & Issuance of Order of Payment. Clients shall proceed to concerned offices for signature.
4.   Assessment & Payment of Taxes. MTO Staff reviews and approves assessment; and affixes her signature on the printout form.
5.   Signing and Recommending Approval by BPLO and forward to the Mayors’ Office. BPLO will sign & recommend for approval of the LCE.
6.   Approval and Signing of the Mayor & Release of Mayors’ Permit. Mayor approves & releases the Business permit.

Fees             :         Varies/depends on Business Applied (Local Tax Code)
Duration        :         2 hours or within a day

SECURING MAYOR’S CLEARANCE/CERTIFICATION
From MONDAYS to FRIDAYS, 8AM-5PM

Who may avail?       General Public/Residents of Banaybanay

What are the requirements?
*      Original Copy of Police Clearance
*      Latest Community Tax Certificate (CEDULA)
*      Official Receipt from the Mun. Treasurer’s Office

Steps:
1.   Filing/Recording of Request. Submit documents required to in-charge.
2.   Preparation of Clearance or Certification. MO staff prepares the Clearance or Certification.
3.   Approval of the Mayor. Mayor approves the Clearance or Certification. 
4.   Release of Clearance or Certification. The Clearance or Certification is released to the client. Client leaves 1 copy with the Records/Archives Section for file. 

Fees             :         P50.00
Duration        :         25-30 minutes, may vary depending on Mayor’s availability
